Fees will vary based on the service requested and time required for appointment and to complete required documentation. My fee schedule of the specific services you are requesting will be provided during a free screening call prior to scheduling our appointment.
Insurance & Private Pay
I am credentialed with Aetna, Cigna & Optum/United Health Care in my private practice. Please note, I am also in-network with several insurance companies as part of other work I do and this insurance does not apply to my private practice clients.
If you are a private pay client and would like to bill insurance, please contact your insurance provider to learn whether I will be accepted as an out-of-network provider. I offer superbills that can be submitted to your insurance for reimbursement. Full payment is required at the time of service.
No Surprises Act
Effective January 1st, 2022, the No Surprises Act, Title 45 Section 149.610 of the Federal Regulations, has created new obligations for healthcare providers, facilities, plans, and insurers that are intended to protect patients from receiving unexpected or “surprise” medical bills. One obligation under this Act is for healthcare providers to provide all uninsured and self-pay clients with a Good Faith Estimate of expected charges. You may ask for this estimate when scheduling services, and you have the right to receive a Good Faith Estimate at least 1 business day before beginning services. You also have the right to dispute any bills that are at least $400 more than your Good Faith Estimate.
